Forms
To help make completing forms easier and faster, we have recently begun to save several forms in Adobe Acrobat format. This allows you to complete a form on-line without losing any of the formatting, print the number of copies desired and send to the Benefit Trust Fund office.
Your computer may already have the Acrobat Reader program. If not, the current version of Acrobat (5) is available free from Adobe's website. Download the Acrobat Reader and then point and click on any of the PDF forms found below, complete them on-line by filling in the blanks, print and send to the Benefit Trust Fund office. |
