1. Your Summary of Benefits -
This is a listing (compiled October 2011) of the voluntary programs/plans you are enrolled in through the Trust.
2. The 2010 Summary Annual Report Three pages.
Describes the financial health of the Benefit Trust Fund and the Legal Defense Fund
3. SMM Booklet - Legal document detailing internal changes on how the Benefit Trust has re-structured its plans/programs. You should not notice any changes to the actual benefits provided through the Trust.
4. Beneficiary Change Form - Use this form to change your insurance beneficiary. This is a self mailer. Just fold, seal and return – no stamp needed.
