Now all plan members get On-the-Job / Off-the-Job coverage 24/7, and the cost is going DOWN. Full coverage will be $65 monthly.
The The CCPOA Disability Benefit Plan is designed to help you with basic living expenses while you are unable to work. 
You have a challenging job with more than a fair share of risk and exposure to harm. To protect yourself and your loved ones, the CCPOA Benefit Trust Fund offers a disability benefit if you are seriously injured or ill and out of work for an extended period. The CCPOA Disability Benefit Plan is a voluntary program. You pay a small monthly premium for either Gold or Silver Shield coverage. In exchange, you have the peace of mind knowing that you can protect yourself and meet your basic living expenses while unable to work.
A disability that prevents you from working can have a major impact on your life. Disability coverage can provide you with a source of income while you’re disabled and unable to work.
The Disability Benefit Plan provides benefits if you are unable to work due to a disability that is covered under the Plan.
- Gold Shield Complete – for both short and long-term illnesses and injuries caused while on-or-off the job.
- Silver Shield Coverage – for your short-term illnesses or injuries (up to 24 months). This program has 180 day elimination period.
If you are currently enrolled in Silver Shield, and would like to upgrade to Gold Shield Complete with the on-the-job coverage, please fill out and submit the proper form below:

IF YOU ARE CURRENTLY 
RECEIVING DISABILITY FUNDS:
Members who are currently receiving a payout from an existing Disability Claim have the option of using those funds to pay for CCPOA dues or other Trust programs.
These funds can be automatically diverted by notifying the Trust.

Silver Shield is $45 monthly.
